14POSITION TITLE: ISFC Social Worker
PRIMARY SUPERVISOR: Foster Care Supervisor or Administrator
DEPARTMENT: Intensive Services Foster Care
SUPERVISES: N/A
STATUS: Full-Time, Non-Exempt
SALARY RANGE: $28.00 – $35.00 per hour
Compensation will be commensurate with education, experience, licensure, and based on appropriate industry standards.
ESTIMATED HOURS: 40 hours per week, provides 24/7 support to youth and foster families, as needed

POSITION DESCRIPTION: The ISFC Social Worker is responsible for the supervision of the treatment plans for each youth assigned to Saving Innocence’s Intensive Services Foster Care Program. The ISFC Part Time Social Worker is responsible for working with the child, resource family, birth family, significant others, county, advocate, and adjunct treatment persons to enable the child to attain their treatment goals. The position requires that the individual be knowledgeable of, and be in compliance with the applicable laws and regulations of the client population served and the programs provided. The individual must have the ability to work effectively with others, manage personnel and program issues as they apply to the position.
REQUIREMENTS & QUALIFICATIONS:
- A Master’s Degree is required in a field approved by the State Department of Social Services, Community Care Licensing Regulations or
- A Bachelor’s degree in social work, or other behavioral science from an accredited college or university and three years of experience in a child welfare program or two years of employment in a licensed FFA. They may be granted an exception provided they complete the coursework and field experience requirements specified above.
- Bilingual English/Spanish preferred.
- Must possess a valid California Driver’s License.
- Statement of sound physical health from a licensed physician, including tuberculin skin test.
- Fingerprint clearance from the Department of Justice, FBI and Child Abuse Index.
- Must meet all requirements set by Community Care Licensing pertaining to the hiring of employees by a licensed community care facility.
- Appropriate experience and education in a related field:
- Ability to establish rapport with children and families as well as work effectively within the agency and the social work community at large.
- Experience working with CSEC population, preferred.
- Professional licensure, preferred (LMSW, LMFT, LPCC, etc.)
DUTIES & RESPONSIBILITIES:
All of the responsibilities listed below are considered essential functions of this position. Additionally, this position is expected to ensure that the core operating values and the strategic direction of the agency are adhered to.
Daily
- Maintains a log of follow-up contact in the child’s file.
- Does daily phone check-ins with resource family and youth during the first week of joining.
Weekly
- Provide in-home training and support to the resource families during weekly meetings.
- Documents child’s weekly progress at home, school, and/or work, major incidents, family time and other significant, collateral contacts.
- Provide weekly family consultation visits for each child in the resource family and work closely with the ISFC treatment team at least once per week.
- Provides weekly consultation to the resource parent, regarding the child’s treatment. Consultation will consist of discussion of a child’s progress towards goals as defined in the Needs and Services Plan, as well as teaching and discussion of therapeutic interventions, positive disciplinary techniques and resource parent training.
- Provides weekly face-to-face family consultations with each child to accomplish treatment goals, minimum of one of which shall be at the Resource Family Home. If a child has needs that require the services of an outside therapist for a particular issue, the social worker will collaborate with the therapist. This collaboration is necessary for purposes of consistency and support of the written treatment goals.
- Meet at least once per week with the ISFC Program Administrator or designated supervisor.
- Chart weekly progress notes and maintain all client files up to date within 7 business days.
Monthly
- Collects records of allowances, medical and behavioral appointments for all children over four years old from the family.
- Ensure all collected records are filed on the youth’s case within 7 business days.
Quarterly
- Design and implement each child’s treatment plan in conjunction with the In-Home Support Counselor, Case Manager, Therapist, or any other service provider in the youth’s team.
- Develops an Appraisal/Needs and Service Plan with input from the CSW or DPO, the child, the resource family, and birth parents (if appropriate and available).
- Arranges, prepares, and leads all Initial and Quarterly Needs and Services Plans, and completes the full report within five (5) working days prior to the due date for supervisor’s review. Following supervisor’s approval, copies of these reports are to be sent immediately to the county, and outside therapist (if involved). Goals are to be given to and discussed with the resource family.
On-going
- Schedules and conducts a matching process for new youth joining SI resource families.
- Assist in screening, training and selection of resource families.
- Reviews children’s file upon receipt.
- Completes the intake assessment of the children.
- Completes the Intake Packet forms and gives copies of signed forms to all persons who signed the forms. Forms that have yet to be signed by parent/guardian, child, and/or placement worker will need to be mailed out immediately.
- Function as the ICC (Intensive Care Coordinator) team leader and coordinate all treatment and placement activities.
- Coordinates CFT meetings with treatment team and informal supports.
- Conducts intake meetings with child, birth family (if available), resource family and CSW or DPO.
- Functions as liaison with the county, the child’s therapist, family, school personnel, supervisor and/or any other adjunct treatment team members. Provides all necessary paperwork for a child’s school enrollment and participation in therapy and other activities.
- A continual assessment of joining will be done to determine if the FFA can meet the child’s service needs.
As Needed
- Coordinate all activities involving the birth parents (including counseling, court appearances, home visits and reunification efforts).
- Attend all program staff meetings, treatment reviews, IEPs and other important or agency meetings as necessary.
- Facilitate and implement the Legal Permanency Plan; The continued development and implementation of the Child’s Needs and Services Plan, including the Transitional Independent Living Plan.
- Obtains approval from CSW or DPO prior to allowing any contact between the child and others. Approves and facilitates all ongoing family time with children.
- Encourages resource parents to involve children in community activities and extracurricular activities.
- Coordinates child’s separation report with CSW or DPO, resource family and parent/guardian. Provides transportation if necessary.
- Submits a separation report to the ISFC Supervisor within five (5) days.
- Submits all incident reports, home assessments and any other documentation to the supervisor for approval.
- Completes staff training as required by the agency and CCL, DCFS, and Probation.
- Work with no more than 4 ISFC youth as a PT social worker and 8 ISFC youth as a full time social worker.
- Ensures educational needs of the children are being met by attending school meetings and IEP meetings.
- Assists supervisor with training of new staff.
- Prepares materials, attends and teaches on-going resource parent trainings as scheduled.
- Attends and/or facilitates resource parent training and support groups as directed by the supervisor.
- Assists with administrative duties within the ISFC Program.
- Communicates with new prospective resource families to assist during the pre-approval process.
- Adheres to all legal parameters for ensuring and maintaining clients’ confidentiality including HIPAA, State and Federal laws.
